Precision care for spine and joint pain—without starting with surgery

Deborah Westergaard, MD reviews your imaging, evaluates you and performs image-guided procedures, including PRP and bone marrow concentrate. In Dallas and Plano, care starts with your diagnosis and treatment options.

HOW THE DECISION GETS MADE

The right treatment begins with identifying the likely pain source

Most people arrive with an x-ray or MRI scan and a recommendation. The scan is one input, not the answer.

Deborah Westergaard, MD reviews your imaging with you, examines you, and uses ultrasound in the room when it clarifies the picture. What she recommends depends on what the examination shows, not on what the report is called.

That may be an orthobiologic procedure, a conventional interventional procedure, shockwave therapy, rehabilitation, a referral to another specialist—or no procedure at all. She performs the procedures herself.

Diagnosis first

Recommendations follow an appropriate medical evaluation.

Precise guidance

Applicable procedures use ultrasound or fluoroscopy.

Candid options

Benefits, limitations, alternatives, and next steps are discussed.

TREATMENT OPTIONS

Services selected around your condition—not a predetermined procedure.

Not every treatment is appropriate for every patient. Suitability depends on diagnosis, health, examination, imaging, treatment history, and goals.

Orthobiologic procedures

Platelet-rich plasma (PRP) or bone marrow concentrate may be considered for selected joint, tendon, ligament, or spine-related conditions. These procedures use material obtained from the patient and are performed with image guidance when appropriate.

  • Knee, hip, shoulder, and other joint conditions

  • Selected tendon and ligament injuries

  • Selected spine-related structures

Orthobiologic procedures do not guarantee tissue restoration or avoidance of surgery. Outcomes vary, and evidence differs by condition and procedure.

Precision spine and joint injections

Established interventional procedures may help clarify a suspected pain source, reduce symptoms, or support participation in rehabilitation. The specific approach depends on the structures believed to be involved.

  • Epidural injections

  • Facet and sacroiliac joint procedures

  • Selected nerve blocks and hydrodissection

  • Other image-guided joint procedures

An injection may provide diagnostic information or symptom relief, but duration and degree of benefit vary.

OrthoShockwave therapy

Shockwave therapy uses focused acoustic energy and may be considered for selected chronic tendon, ligament, muscle, or joint-related conditions.

  • Selected elbow and shoulder tendon conditions

  • Plantar fasciitis and other foot concerns

  • Selected hip, knee, and soft-tissue conditions

The number of sessions, expected response, discomfort, and alternatives should be discussed after evaluation. Results vary.

Different tools serve different purposes

Support repair

Selected orthobiologic or shockwave approaches may be considered when the diagnosis and evidence support their use.

HOW OPTIONS DIFFER

Clarify the diagnosis

Some procedures are used partly to test whether a particular structure is contributing to pain.

Manage symptoms

Some treatments aim to reduce symptoms and help patients participate more fully in rehabilitation.

HOW OPTIONS DIFFER

A procedure should fit the person as well as the diagnosis

An evaluation may be reasonable if…
A procedure may not be the next step if…
  • Symptoms suggest an urgent or progressive neurological problem

  • Infection, fracture, cancer, or another serious condition is suspected

  • The proposed treatment does not match the diagnosis

  • Surgical consultation or another specialist is more appropriate

  • Pain continues despite appropriate conservative care

  • You want to understand non-surgical options

  • Your diagnosis or pain source remains unclear

  • You want a physician to review prior imaging and treatment
